Clonal mast cell activation syndrome with anaphylaxis to sulfites.

چکیده
Sulfites are rarely suspected as causative agents of immediate-type hypersensitivity. We report on a 49-year-old male patient who developed recurrent severe hypotension after food ingestion. A diagnosis of monoclonal mast cell activation syndrome was established. In the double-blind, placebo-controlled food challenge, the patient reacted to potassium metabisulfite with anaphylaxis.

Insights in Anaphylaxis and Clonal Mast Cell Disorders
The prevalence of anaphylaxis among patients with clonal mast cell disorders (MCD) is clearly higher comparing to the general population. Due to a lower frequency of symptoms outside of acute episodes, clonal MCD in the absence of skin lesions might sometimes be difficult to identify which may lead to underdiagnosis, and anaphylaxis is commonly the presenting symptom in these patients. Although...
متن کاملDiagnostic value of tryptase in anaphylaxis and mastocytosis.
Serum (or plasma) levels of total and mature tryptase measurements are recommended in the diagnostic evaluation of systemic anaphylaxis and systemic mastocytosis, but their interpretation must be considered in the context of a complete workup of each patient. Total tryptase levels generally reflect the increased burden of mast cells in patients with all forms of systemic mastocytosis (indolent ...
متن کاملA novel combination of an IgE mediated adult onset food allergy and a suspected mast cell activation syndrome presenting as anaphylaxis
BACKGROUND Adult onset food allergy is a rare, but increasingly recognized phenomenon. Mast cell activation syndromes present an ongoing diagnostic and classification challenge. The combination of the two has been rarely described in the literature. CASE PRESENTATION We present a case of a new onset, IgE mediated food allergy in combination with a mast cell activation syndrome in an elderly p...
متن کاملRecent advances in mast cell clonality and anaphylaxis
Clonal expansion of mast cells carrying the D816V c-kit mutation results in mastocytosis. Recent studies identified the presence of clonal mast cells carrying this mutation in patients with anaphylaxis without classic diagnostic findings of systemic mastocytosis.
متن کاملExpanding spectrum of mast cell activation disorders: monoclonal and idiopathic mast cell activation syndromes.
BACKGROUND In recent years, 2 new syndromes of mast cell activation have been described in patients with episodes of mast cell mediator release that range from flushing and abdominal cramping to anaphylaxis: monoclonal mast cell activation syndrome (MMAS) and idiopathic mast cell activation syndrome (MCAS). OBJECTIVE This review will discuss these 2 new syndromes in the larger context of mast...
